Care Manager-Women's Residential

The Case Manager for our Women's Residential Services will provide case management for clients in the residential program who may be diagnosed with chemical dependency or co-occurring chemical dependency and mental health disorders.

Qualifications:

  • Requires a minimum of a bachelor's degree in a related field and 2-3 years of experience in mental/behavioral health or substance use recovery institutions.
  • Requires individuals to be at least 21 years of age or older to be a listed driver with the organization, as well valid Florida driver's license and current vehicle insurance coverage.


Essential Responsibilities:

  • Conduct client intake, screening, assessment, orientation, and related reporting.
  • Develop and implement case management plans for clients that address goals of obtaining stable housing, gainful employment, engagement in behavioral health recovery support, and linkage to ancillary services to support those goals.
  • Develop transition and discharge plans with clients that include provisions for behavioral health disorders.
  • Coach clients with treatment plans and self-help recovery services, individualized health and wellness goals, and self-responsibility.
  • Facilitate life skills group with fidelity to evidence-based practices.
  • Prepare and maintain clinical records in accordance with policy and procedure.
  • Interface positively with community partners.
  • Participate in client case reviews and team meetings.
  • Make referrals to appropriate resource agencies and document outcomes of referrals.
  • Provide crisis intervention when needed.
  • Assist Behavioral Health Techs in transporting clients as required.
