Counselor- Lakeland, FL

The Counselor will provide clinical intervention and case management for patients at risk for or diagnosed with substance use and/or co-occurring mental health disorders. Our MAT program combines FDA-approved medications (Methadone or Subutex) with counseling, support services, and medical care to give patients the best chance at lasting recovery.

POSITION REQUIREMENTS

  • Master's degree in a related field of study with knowledge of the 12-step program, behavior modification, case management, addiction, and COD. Ability to read and interpret documents such as safety rules, operating instructions, 
  • Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Ability to speak and communicate effectively. 
  • Effective written and verbal communication is required for reporting. Ability to solve practical problems and deal with a variety of concrete variables in situations where limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. 
  • Ability to demonstrate and model appropriate boundaries with patients. 
  • Evaluate mental health and substance use diagnosis and create and implement a treatment plan. Ability to multitask and prioritize tasks. Must be motivated and interested in working with patients who have mental health, substance use, and court involvement.

POSITION RESPONSIBILTIES

  • Conduct patient assessments and evaluations to develop and implement treatment plans.
  • Provide case management-type services for patients.
  • Provide individual, group, and family counseling as necessary in person and using telehealth.
  • Prepare and maintain clinical records in accordance with 65 D-30.
  • Submit reports as required.
  • Participate in clinical consultation and treatment team meetings.
  • Make referrals to appropriate resource agencies.
  • Act as a liaison between resource agencies. Collaborate with all internal departments to maintain treatment continuity when applicable.

WORK ENVIRONMENT

General office, health care setting. Most work is completed while sitting at a desk using standard office equipment. Fast-paced, team approach. Multi-tasking requires above-average time management skills. Team approach.
