The Counselor for the MAT (Medication Assisted Treatment) program will provide clinical intervention and case management for patients at risk for or diagnosed with substance use and/or co-occurring mental health disorders.

REQUIIRED EDUCATION & SKILLS
- Minimum of Bachelor's degree (master's degree preferred) in a related field of study with knowledge of the 12-step program, behavior modification, case management, addiction, and COD. Ability to read and interpret documents such as safety rules, operating instructions,
- Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Ability to speak and communicate effectively. Effective written and verbal communication is required for reporting.
- Ability to solve practical problems and deal with a variety of concrete variables in situations where limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Ability to demonstrate and model appropriate boundaries with patients.
- Evaluate mental health and substance use diagnosis and create and implement a treatment plan. Ability to multitask and prioritize tasks.
- Must be motivated and interested in working with patients who have mental health, substance use, and court involvement.
ESSENTIAL RESPONSIBILITIES:
- Conduct patient assessments and evaluations to develop and implement treatment plans.
- Provide case management-type services for patients.
- Provide individual, group, and family counseling as necessary in person and using telehealth.
- Prepare and maintain clinical records in accordance with 65 D-30.
- Submit reports as required.
- Participate in clinical consultation and treatment team meetings.
- Make referrals to appropriate resource agencies.
- Act as a liaison between resource agencies. Collaborate with all internal departments to maintain treatment continuity when applicable.
- Provide crisis intervention when appropriate
- Participate in staff development and team-building activities.

- Provide case management-type services for patients.

WORK ENVIRONMENT
General office, health care setting. Most work is completed while sitting at a desk using standard office equipment. Fast-paced, team approach. Multi-tasking requires above-average time management skills. Team approach.
PHYSICAL DEMANDS
Abilities in reaching, bending, talking, sitting, carrying, standing, grasping, fine hand coordination, the ability to hear, the ability to read and write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 15 pounds. May be required to deliver standing presentations, speak, and listen to the audience.
