The LPN will assess the medical needs of residential clients and assist in medical care based on a systematized body of knowledge. Utilizes nursing process, including assessment, identification of nursing problems, planning, execution, evaluation, and modification of nursing interventions as determined by patient outcomes. Acknowledges, coordinates, and implements diagnostic and therapeutic orders. Assures patient safety through safe administration of medications, patient injury prevention, and infection control. Participates as a contributing member of the multidisciplinary treatment team.

QUALIFICATIONS
- Licensed Practical Nurse program with 1 year of experience preferred.
- Current, unrestricted license to practice as a Licensed Practical Nurse in the State of Florida.
- Ability to effectively present information and respond to questions from managers, patients, families, employees, and the general public. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists, and request the assistance of the RN when necessary.
- Ability to multitask, display decision-making and critical thinking skills, and basic computer skills.
ESSENTIAL DUTIES

- Complete drug and past patient medical history, monitor vital signs, and complete nursing assessment upon admission and as needed.
- Coordinate with the medical director and nurse practitioner for new client physicals, ensure all client medications are obtained, and update the client's medication sheet as necessary.
- Screen clients for co-occurring disorders and refer to the psychiatric clinic, and arrange client HIV education, testing, and counseling.
- Administer client medication and observe client health status, triage client medical complaints.
- Provide medical case management for client visits to outside resources.
- Prepare and maintain clinical and program records.
- Participate in clinical staffing and team meetings.
- Schedule client clinic appointments.
- Promote and encourage good communication with counseling staff to ensure optimal holistic care of clients.
- Conduct educational groups health related topics, including co-occurring disorders.
- Communicate with other medical personnel in case of emergencies.
- Maintain facilities medications.
- Transcribe the physician's orders.
- Submit reports as required.
PHYSICAL DEMANDS
Abilities in reaching, bending, talking, sitting, carrying, standing, grasping, fine hand coordination, the ability to hear, the ability to read and write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. May be required to complete standing presentations involving speaking and listening to the audience.
