Licensed Practical Nurse- Outpatient

The Outpatient Nurse will provide direct patient treatment through thorough assessment of the medical needs of residential clients and assisting in the provision of proper medical care with a systematized body of knowledge. Utilizes nursing process, including assessment, identification of clinical concerns, planning, execution, evaluation, and modification of nursing interventions as determined by patient outcomes. Acknowledges, coordinates, and implements diagnostic and therapeutic orders. Ensure patient safety through safe administration of medications, prevention of violent behavior, patient injury, and infection control. Participates as a contributing member of the multidisciplinary treatment team. Participates in Quality Improvement activities.

ESSENTIAL RESPONSIBILITIES

  • Perform a thorough patient assessment to include drug and past medical history, vital signs, and complete final nursing assessment upon admission and as needed.
  • Obtain all client medication and update the client's medication sheet as needed, screen clients for co-occurring disorders, and refer to the Emergency Acute Crisis Stabilization facility for psychiatric and/or behavioral disorders.
  • Administer client medication and observe client health status, as well as triage client medical complaints.
  • Provide medical case management for client visits to external resources.
  • Schedule client medical appointments with TGH Clinic and /or additional clinic appointments with internal and external partners.
  • Promote and encourage good communication with case management staff to ensure optimal holistic care of clients.
  • Arrange client HIV education, testing, and counseling.
  • Conduct educational groups health related topics, including co-occurring disorders.
  • Communicate with other medical personnel in case of emergencies.
  • Maintain facilities medications.
  • Transcribe the physician's orders.
  • Participate in client staffing and team meetings and submit reports as needed per program requirements. reports as required. Prepare and maintain clinical and program records.


QUALIFICATIONS:

Licensed Practical Nurse or Registered Nurse program with 1 year of experience preferred. Current, unrestricted license to practice as a Licensed Practical Nurse or Registered Nurse in the State of Florida. Ability to effectively present information and respond to questions from managers, patients, families, employees, and the public. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists, and request the assistance of the Nurse Supervisor or Nurse Manager when necessary. Ability to multitask, display decision-making and critical thinking skills, and basic computer skills. and other stakeholders.
