Assessment Specialist PRN-Outpatient

POSITION SUMMARY:

This is an advanced professional position that performs initial reassessments to evaluate client's needs for

appropriate treatment including but not limited to mental health, substance abuse, and primary care.

Assessments will include psychosocial assessments and treatment planning for the following populace:

children, adolescents, and adults. The Assessment Specialist will be responsible for maintaining patient care

by program guidelines and external standards (Joint Commission, Medicaid, etc.).

QUALIFICATIONS:

Must have a minimum of a Master's degree in a human services-related field with 2 years of experience.

Registered Interns and/or Florida License (LMHC, LCSW, LMFT) is preferred. Bilingual (English and Spanish) is a

plus. Must know the Diagnostic and Statistical Manual of Mental Disorders and Medicaid documentation

requirements. Computer knowledge should include Microsoft applications (Word, Excel) and Electronic Medical

Records experience. helpful. May provide clinical oversight in a multidisciplinary team.

PHYSICAL DEMANDS:

Abilities in reaching, bending, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination,

ability to hear, ability to read and write, and the ability to remain calm under stress. Ability to reach with hands

and arms, climb stairs, balance, stoop, kneel, crouch, or crawl. Specific vision abilities include close, distance,

color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds of dead

weight alone. May be required to complete standing presentations.

WORK ENVIRONMENT:

General office and health care setting. Most work is completed while seated using standard office equipment.

Multi-tasking, fast-paced, team approach requiring above-average time management skills.

FUNCTIONAL RESPONSIBILITIES INCLUDE:

- Conduct thorough assessments and develop service plans, treatment plans, and discharge plans.

- Accurate and timely documentation of clinical and administrative services following program guidelines

and external standards (Joint Commission, Medicaid, etc.).

- Meets productivity requirements for the designated program through efficient workflow management.

- Maintains cooperative relationships with other internal departments and external agencies and keeps

supervisor informed at all times of all relevant clients, program, and community issues.

- Understands the basic tenants of Co-Occurring disorders and the relationship between mental health

and substance abuse. The assessment Specialist will also demonstrate knowledge of the age-specific

needs of the clients treated in the program.

- Other duties as assigned or needed as per program requirements.

COMPLIANCE RESPONSIBILITIES INCLUDE:

- Adheres to all applicable federal, state, local, and company-maintained standards of compliance,

ethics, and policies concerned with the administration and delivery of agency services.

- Meets deadlines, requirements, and expectations as established by the supervisor, agency, and

department. Provides oversight, auditing, and reporting.

- Completes administrative functions and data entry timely and accurate.
