Care Coordinator- Outpatient



Perform initial consultation to determine client-specific needs and goals. Educates clients on Agency roles and treatment options available. Develop care plans and Coordinate internal and external substance use, mental health, and/or physical health services

QUALIFICATIONS

Bachelor's degree in a Human Services-related discipline and at least 1 year of experience in the mental health field. Ability to read, analyze, and interpret clinical records, procedures, or governmental regulations. Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders. 

FUNCTIONAL RESPONSIBILITIES

  • 1. Perform initial consultation to determine client-specific needs and goals
    2. Educate clients on Agency roles and treatment options available
    3. Develop care plans in coordination with Agency providers and treatment options
    4. Coordinate internal and external substance use, mental health, and/or physical health services
    afforded to each client in support of the individualized care plan
    5. Consult with client care team to evaluate assigned interventions/treatment to ensure congruity with
    continued care plan
    6. Continually review care plan with client to ensure appropriate treatment and education as client
    progresses
    7. Initiate care plan updates and additional interventions when appropriate for client progress
    8. Provide crises intervention, as needed
    9. Submit reports and complete documentation as required

    PHYSICAL DEMANDS
  • Abilities in reaching, bending, talking, sitting, carrying, standing, grasping, fine hand coordination, the ability to hear, the ability to read and write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch, or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. May be required to complete standing presentations.
  • WORK ENVIRONMENT
  • High-paced, collaborative team performance with the emphasis on critical thinking and decision-making with guidance from the program supervisor/manager. Climate-controlled office environments with frequent exposure to clients. Some programs may require traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods.
