Care Manager-Community Housing (FT)

The Care Manager for Community Housing will provide case management for clients in the homeless program who may be diagnosed with chemical dependency or co-occurring chemical dependency and mental health disorders. The Community Housing Solutions Center (CHSC) is a place where individuals can get a clean, safe bed, warm meals, and build a life of recovery. CHSC has been the place where that chapter ended and a new one began, helping individuals move into their own apartment and a paycheck of their own.

POSITION REQUIREMENTS

Bachelor's degree in a Human Services-related discipline and at least 2 years of experience in a related field. Ability to read, analyze, and interpret clinical records, procedures, or governmental regulations. Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Knowledge of homeless services preferred: 12-step program, behavior modification, case management, and addiction. Must have a valid Florida Driver's License, reliable transportation, and vehicle coverage for routine client visits or local transportation assistance within Hillsborough County.

ESSENTIAL RESPONSIBILITIES

  • Conduct client intake, screening, orientation, assessments, evaluations, and provide crisis intervention when needed.
  • Develop and implement case management plans for clients that address goals of obtaining stable housing, gainful employment, engagement in behavioral health recovery support, and linkage to ancillary services to support those goals.
  • Develop transitions and discharge plans with clients that include provisions for housing, employment, and behavioral health disorders.
  • Provide case management services for clients and participate in client staffing and team meetings.
  • Facilitate life skills group with fidelity to evidence-based practices.
  • Prepare and maintain clinical records within the required timeframe.
  • Interface positively with community partners.
  • Submit reports as required.
  • Make referrals to appropriate resource agencies and document outcomes of referrals.
  • Assist in training and education of other staff members, interns, and students.

Residential Care Housing facility in a team-oriented environment. Fast-paced setting, especially during high-peak enrollment periods or discharge

PHYSICAL DEMANDS

Abilities include reaching, bending, talking, sitting, carrying, standing, grasping, fine hand coordination, hearing, reading, writing, and staying calm under stress. Must be able to reach with hands and arms, climb stairs, maintain balance, stoop, kneel, crouch, or crawl. Vision abilities required include close-up, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 50 pounds alone. May be required to deliver standing presentations that involve speaking and listening to the audience
