Care Manager-HCPS MRT

SUMMARY

The Care Manager for our Hillsborough County Public School Mobile Crisis Team will provide advocacy, coordination and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication. The Hillsborough County Public Schools Mobile Response Team (HCPS MRT) at Ibis Healthcare represents a vital partnership between our clinical experts and the school system, designed to provide immediate, compassionate crisis intervention for students experiencing mental health emergencies on school campuses throughout Hillsborough County. This comprehensive approach ensures that students receive thorough, professional assessment and appropriate intervention while maintaining their safety and connection to educational support systems. The Care Manager participates as a member of a multidisciplinary treatment team and reports to the unit/department supervisor.

POSITION REQUIREMENTS

  • Minimum of a Bachelor's degree in a Human Services related discipline and at least 1 year experience in the mental health field. (Child welfare programs require 2 years experience with at risk populations and a child welfare certification. ) 
  • Must have reliable transportation, valid Florida driver's license and insurance.
  • Ability to read, analyzes, and interprets clinical records, procedures, or governmental regulations. Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders.

RESPONSIBILITIES

  • Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
  • Complete all clinical and administrative documentation based on established timelines as per program requirements.
  • Communicate effectively and maintain cooperative relationships with internal and external resources.
  • Develop service plans, treatment plans and/or aftercare plans with clear goals and objectives as per program requirements. 
  • Maintain efficient workflow management and organization as per program requirements.
  • Maintain proficient quality of clinical documentation as per program requirements.
  • Other duties as assigned or needed as per program requirements

WORK ENVIRONMENT

High-paced, collaborative team performance with the emphasis on critical thinking and decision-making with guidance from program supervisor/manager. Climate controlled office environments with frequent exposure to clients. Some programs may require traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services or onsite clinical programs in a variety of socioeconomic neighborhoods
