Care Manager-MRT Dispatcher

POSITION OVERVIEW?

The Care Manager Dispatcher will provide direct care to individuals served with behavioral health needs, including supporting individuals served with coordination and continuity of evidence-based care for individuals and families in need. As a member of our Adult Crisis Mobile Response Team, the Care Manager Dispatcher will work through a collaborative triage, assessment, planning, and communication process to serve individuals in crisis. Our Mobile Crisis Response Team brings expert mental health crisis intervention directly to adult, children, and families throughout Hillsborough County, providing immediate support to client's home, or within the community. The position requires traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services in a variety of socioeconomic neighborhoods.

JOB REQUIREMENTS:

  • Must have a Bachelor's Degree in Human Services or a related field of study in Human Behavior /Science.
  • Must be able to lift and carry up to 50lbs of dead weight
  • Must have excellent interpersonal skills
  • Valid Florida Driver's license, reliable personal transportation, and insurance.

POSITION RESPONSIBILITIES

  • Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
  • Complete all clinical and administrative documentation based on established timelines as per program requirements.
  • Communicate effectively and maintain cooperative relationships with internal and external resources.
  • Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements.
  • Maintain efficient workflow management and organization as per program requirements.
  • Maintain proficient quality of clinical documentation as per program requirements.
  • Other duties as assigned or needed as per program requirements.

WORK ENVIRONMENT:

  • Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe supportive environment.
  • This is a 60-bed Adult Crisis inpatient served care facility.
  • Office setting within the Crisis center ?
  • Must be capable of lifting 50 pounds of dead weight alone.
