The Pregnancy Specialist is responsible for delivering comprehensive case management services and facilitating access to community resources for pregnant and post-partum women.

QUALIFICATIONS

A minimum of a High School Diploma/GED, Bachelor's degree preferred in a related field of study, preferably in a Human Services discipline, and 2-4 years of experience in the related field. State of FL registered interns with clear/active status preferred. Bilingual (in English and Spanish) is preferred but not required. Computer knowledge should include proficiency in Microsoft applications (Word, Excel, Teams) and experience with Electronic Medical Records. Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Must possess excellent communication skills. Ability to solve practical problems and deal with a variety of concrete variables. May require a valid Florida Driver's license and reliable transportation for external training and client visits. Knowledge of the 12-step program, behavior modification, case management, and addiction.

POSITION RESPONSIBILTIES

• Maintain the assigned caseload as determined by the Counselor Supervisor.
• Create, update, and document intervention plans for assigned patients using the EHR.
• Connect patients with relevant community resources and document the outcomes in the EHR.
• Maintain a current log of appropriate referral sources for pregnant and post-partum women.
• Attend weekly pregnancy treatment team meetings and record notes.
• Serve as the designated agency representative in community meetings as required. Act as a liaison between community partners, resources, and the organization
• Complete monthly reporting for various funder requirements.
• Maintain documentation in accordance with regulatory standards.
• Participate in clinical staffing and other organization treatment team meetings, as indicated.
• Provide and assist in coordinating patient transportation to community appointments and events.
• Conduct thorough screenings for child development and provide referrals as necessary.
• Conduct pre- and postpartum depression screenings with participants served.
• Participate in both internal and external training programs to enhance knowledge and provide the best assistance to the participants we serve.

WORK ENVIRONMENT

General office and health care setting. May be required to travel within Hillsborough County via agency vehicle or personal vehicle to provide services in a community setting. Registered drivers with the organization must have a valid Florida driver's license, possess valid insurance vehicle coverage, and be a minimum of 21 years of age. Most work is completed while sitting using standard office equipment. Multi-tasking, fast-paced, team approach requiring above average time management skills.

PHYSICAL DEMANDS

Abilities include reaching, bending, talking, sitting, carrying, standing, grasping, fine hand coordination, hearing, reading, writing, and staying calm under stress. Must be able to reach with hands and arms, climb stairs, maintain balance, stoop, kneel, crouch, or crawl. Vision abilities required include close-up, distance, color, peripheral, depth perception, and the ability to adjust focus. Must be capable of lifting 25 pounds alone. May be required to give standing presentations involving speaking and listening to the audience.
